The Center for Integrated Manual Therapies, LLC, is a physical therapy practice that exclusively provides individualized, hands-on care. Karen Johnson, Scott Juzwak, and Kyle McIntyre are licensed physical therapists with over 50 combined years of post graduate training and experience in manual evaluation and treatment techniques that determine the cause of and alleviate pain and improve posture and movement. Our treatment will enhance your ability to perform daily tasks, exercises, athletics and recreational activities. We teach each client self-care and injury prevention techniques.

What to bring and what to expect:
1. Clothing: Men: gym shorts and t-shirt

Women: gym shorts, t-shirt and/or sports bra
All: warm up suit or any loose fitting clothes that you are comfortable wearing

2. Questions/Concerns: Please bring any questions you have about physical therapy and the problem(s) you want us to address.
3. Expect to be with your physical therapist during the entire session. You will receive comprehensive, hands-on evaluation and treatment as well as extensive education about self care, self treatment, prevention and exercise.

4. You can expect improved posture, reduction of pain, education about body mechanics, posture, exercise, performance of daily activities - all with greater efficiency and enhanced feelings of wellness, health and independence. Our treatment is a team effort between you and your physical therapist!

Initial evaluations are 90 minutes in length, plus 15 minutes for you to complete client information forms. Treatment sessions are typically one hour long, although the length of each session is always based on the needs of the client. You can expect to work with the same physical therapist for every appointment, unless we agree that a second opinion would be helpful or if you need a specific appointment time available with a particular therapist. Our fee is $160/hour. The 90 minute initial evaluation is $250.00. Sessions of shorter or longer duration, in 15 minute segments, are prorated accordingly.

Our practice is cash based. This means that we do not participate with any health insurance plans. We are therefore considered "out-of-network" for all health insurance

plans. We have chosen this form of practice so that we can provide one-on-one, quality care, based on the needs of each client rather than being directed by the requirements of any insurance company. Our rates are therefore typically lower than many insurance based practices. This also means that you are responsible for payment at the time of service. We accept cash, check, Visa, Master Card, Discover, and American Express. We will gladly provide you with a receipt after each session, in order for you to pursue reimbursement from your health insurance carrier for these services. We will also provide copies of our office notes, if required by your insurance carrier.

In order to facilitate the reimbursement process, we suggest that you take the following steps:

Call your insurance carrier and explain that you are going to see a physical therapist who is out-of-network and ask if you have coverage for out-of-network physical therapy. If so, ask what this coverage includes and what deductibles apply to your policy. Ask if pre-authorization is needed for physical therapy services. Ask what information is required in order to be reimbursed (i.e. is there a special form that needs to be completed).

Ask if a physical therapy prescription is needed from your physician. Also ask whether physical therapy office notes are required.

Find out if there are any limits for coverage in terms of dollars per year, visits per year, or visits per condition.

We will gladly work with you so that you can receive the reimbursement from your insurance company that your policy provides.
